Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents in line with facility policies
Administer medications and treatments accurately and document thoroughly
Monitor and record vital signs and patient status, escalating changes to the interdisciplinary team
Collaborate with nurses, therapists, and providers to coordinate excellent care
Educate residents and families about care plans and health maintenance in a clear, respectful way
Maintain accurate clinical documentation and comply with regulatory requirements
Assist with admissions, discharges, transfers, and daily nursing tasks to ensure smooth operations

Qualifications

Current LPN license in Pennsylvania or eligibility to obtain licensure promptly
Graduation from an accredited practical nursing program
Basic Life Support (BLS) certification or willingness to obtain it upon hire
Strong communication skills and a team-first attitude
Previous experience in long-term care or skilled nursing preferred but new grads who are eager to learn are encouraged to apply

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

Medication administration and documentation best practices
Resident assessment and monitoring skills
Infection control and safe patient handling
Proficiency with electronic health records and basic charting
Ability to prioritize, multitask, and remain calm in busy clinical situations
Collaborative communication with patients, families, and care teams
