Roles & Responsibilities

• Provide skilled med/surg and rehabilitation nursing care to a 31-bed unit, delivering safe, compassionate bedside care
• Perform wound care and incision dressing changes with attention to detail and documentation
• Support procedures including Foley catheter placement and assist with blood transfusion protocols as directed
• Manage respiratory needs including trach suctioning and breathing treatments when needed
• Assist with safe patient transfers using Hoyer lifts, wheelchairs and walkers to maintain mobility and prevent injury
• Document all care accurately in the EMR (Meditech/CareTools) and communicate effectively with the interdisciplinary team
• Contribute to efficient shift handoffs and help maintain positive patient ratios and continuity of care

Qualifications

• Valid LPN/LVN license (Texas) required
• BLS certification required
• Minimum 1 year Med/Surg or rehabilitation nursing experience
• Experience with Meditech and/or CareTools (EMR) highly preferred
• Competence with wound care, Foley catheter placement, trach suctioning and Hoyer lift transfers
• Ability to work nights and meet weekend requirements, organized and team-oriented

Education

• LPN/LVN diploma or equivalent

Knowledge & Skills

• Med/Surg and rehabilitation nursing practices
• Wound care and incision dressing management
• Patient transfer techniques (Hoyer lift, wheelchair, walker)
• Foley catheter care and basic transfusion support
• Respiratory care basics including trach suctioning and breathing treatments
• EMR documentation (Meditech, CareTools) and medication cart workflows
• Strong communication, teamwork and organizational skills
