Roles & Responsibilities

• Provide skilled nursing care to patients in their homes with compassion and clinical excellence
• Perform thorough patient assessments and monitor vital signs to guide care decisions
• Administer medications and treatments as ordered, ensuring patient safety at every step
• Assist with wound care and chronic disease management to support healing and quality of life
• Educate patients and families on care plans and medications so they feel confident at home
• Maintain accurate and timely documentation in Cerner to ensure continuity of care
• Collaborate with the interdisciplinary healthcare team to deliver coordinated, patient-centered care

Qualifications

• Active California LVN license (required for submission)
• Minimum 2 years of nursing experience
• BLS certification (required)
• Home Health experience preferred
• Strong documentation and communication skills
• Able to manage time effectively and work independently in patient homes

Education

• Completion of an accredited vocational nursing program or equivalent (LVN)

Knowledge & Skills

• Skilled in patient assessment and monitoring of vital signs
• Medication administration and treatment implementation
• Wound care and chronic disease management
• Proficient documentation practices, experience with Cerner preferred
• Strong patient and family education skills
• Time management and independent decision-making in community settings
• Collaborative approach with interdisciplinary teams
