Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and timely
Monitor patient conditions and report changes to the care team
Document care and observations clearly in the patient record
Assist with daily living activities and promote patient dignity
Collaborate with RNs, therapists, and support staff to deliver coordinated care
Participate in facility processes to maintain a safe, comfortable environment

Qualifications

Current, active LPN (Licensed Practical Nurse) license in Pennsylvania
BLS/CPR certification (or ability to obtain upon hire)
Professional, patient-focused attitude with good communication skills
Prior LPN experience in long-term care, skilled nursing, or similar setting preferred
Ability to pass background checks and meet facility onboarding requirements

Education

Vocational nursing program graduate or equivalent LPN diploma/certificate

Knowledge & Skills

Medication administration and accurate charting
Basic patient assessment and vital sign monitoring
Familiarity with electronic health records and documentation best practices
Infection control principles and safe patient handling
Strong teamwork, time management, and prioritization skills
