Shift Type: Rotating schedule including both weekend days, 6:00 AM - 2:30 PM (Day)

Hours per Week: 40

Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Administer medications and treatments safely and accurately following facility policy
Provide compassionate direct care to residents, promoting dignity and comfort
Monitor and document patient status, reporting changes to the interdisciplinary team
Assist with care planning and collaborate with RNs, CNAs, and other staff for coordinated care
Perform timely and thorough documentation in the medical record
Support residents and families with clear communication and patient education

Qualifications

Current LPN license in Vermont (required)
Previous LPN experience in long-term care, skilled nursing, or rehab settings is a plus
Must be local only - this position requires you to live locally
Must be available to work both weekend days as part of the schedule
Must have reliable personal transportation
Ability to pass background checks and required facility screenings

Education

Completion of an accredited LPN/LVN program and active Vermont LPN license

Knowledge & Skills

Strong clinical assessment and medication administration skills
Familiarity with long-term care nursing practices and resident-centered care
Clear, professional communication with patients, families, and care teams
Good documentation and electronic charting skills
Ability to prioritize tasks in a fast-paced care environment
Team-oriented mindset and a calm approach to resident care
