Roles & Responsibilities

Provide compassionate, resident-centered nursing care as a Licensed Practical Nurse
Administer medications and treatments accurately and safely
Monitor and document vital signs, condition changes, and care provided
Assist residents with activities of daily living and mobility needs
Collaborate with the interdisciplinary team to deliver coordinated care
Maintain accurate, timely documentation in the patient record
Follow infection control protocols and facility policies to keep everyone safe

Qualifications

Active LPN license in Pennsylvania
Completion of an accredited practical nursing program (diploma or certificate)
Current basic life support (BLS) may be required by facility
Strong communication skills and the ability to work well on a team
Dependable, patient-focused, and detail-oriented approach to care

Education

Diploma or certificate from an accredited Practical Nursing program
High school diploma or equivalent

Knowledge & Skills

Medication administration and basic wound care knowledge
Patient assessment and observation skills
Accurate clinical documentation and charting
Comfortable working in a long-term care/skilled nursing environment
Good time management and prioritization under typical nursing demands
Familiarity with electronic medical records and care coordination
