Roles & Responsibilities
- Provide compassionate, direct patient care with attention to safety and dignity
- Administer medications and treatments accurately and timely
- Monitor and record vital signs, reporting changes promptly
- Collaborate with nurses and interdisciplinary team members to coordinate care
- Maintain clear, accurate documentation in the patient record
- Assist with admissions, discharges and patient transfers as needed
Qualifications
- Current LPN license (Licensed Practical Nurse) — Pennsylvania license recommended
- Graduation from an accredited practical nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Ability to pass required background checks and health screenings
- Strong communication skills and a patient-centered mindset
Education
- Diploma or certificate from an accredited practical nursing (LPN) program
Knowledge & Skills
- Medication administration and basic clinical assessments
- Wound care and basic restorative nursing skills
- Electronic health record (EHR) charting and documentation
- Effective communication with patients, families and clinical teams
- Time management, organization and infection control principles
