Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents with dignity and respect
Administer medications and treatments accurately and document thoroughly
Monitor patient status, report changes promptly, and collaborate with the care team
Assist with daily living activities while promoting independence and safety
Communicate clearly with residents, families, and interdisciplinary staff
Participate in care planning and follow established policies and procedures
Maintain accurate clinical records and ensure regulatory compliance

Qualifications

Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure preferred
Graduate of an accredited practical nursing program or equivalent
Basic Life Support (BLS) certification required or willing to obtain
Comfortable with medication administration, charting, and direct patient care
Strong communication skills and a collaborative attitude
Previous long-term care or med-surg experience preferred but not required

Education

Diploma or certificate from an accredited practical nursing program (LPN)

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital sign monitoring
Infection control and safety protocols
Electronic health record (EHR) charting and record keeping
Time management and prioritization during shift work
Compassionate patient- and family-centered care
