Roles & Responsibilities

• Provide high-quality, compassionate nursing care to residents in a long-term care setting
• Administer medications and treatments safely and accurately
• Monitor resident conditions and communicate changes to the care team
• Document nursing assessments, interventions, and resident responses clearly
• Assist with activities of daily living while promoting resident dignity and independence
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Participate in care planning and family communication to support resident goals

Qualifications

• Active LPN license in Pennsylvania (or compact/state-appropriate licensure)
• Graduate of an accredited practical nursing program or equivalent
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Previous experience in long-term care, skilled nursing, or similar settings is a plus
• Strong communication skills and the ability to work well on a team
• Up-to-date immunizations and ability to meet facility onboarding requirements

Education

• Practical Nursing diploma or certificate from an accredited program

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Comfortable with resident assessment and reporting clinical changes
• Familiarity with long-term care routines and regulatory requirements
• Good organizational skills and attention to detail
• Compassionate bedside manner and the ability to de-escalate stressful situations
• Basic computer skills for electronic charting and documentation
