Roles & Responsibilities

• Provide compassionate direct patient care and monitor patient condition
• Administer medications and treatments as prescribed and document accurately
• Collaborate with nurses and multidisciplinary team to support care plans
• Perform routine assessments and report changes promptly to clinical leads
• Maintain clear, timely patient records and handoffs at shift change
• Support a safe, clean environment and adhere to infection control practices
• Assist with patient mobility, ADLs, and comfort measures with dignity

Qualifications

• Current LPN (Licensed Practical Nurse) license—PA licensure preferred
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification or willingness to obtain
• Ability to work 8-hour clinical shifts and reliably attend scheduled shifts
• Clear background check and up-to-date immunizations as required by facility
• Strong communication skills and a collaborative, patient-focused attitude

Education

• Graduation from an accredited Practical Nursing program (or equivalent)
• Current LPN license in good standing

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care, vital sign monitoring, and patient assessment
• Familiarity with electronic charting systems (or willingness to learn)
• Time management and prioritization in a fast-paced clinical setting
• Effective communication with patients, families, and care teams
• Adherence to infection control and safety protocols
