Contract Type: Travel Contract

Shift Type: Monday-Friday, 7:00 AM - 3:00 PM

Hours per Week: 36

Candidate Pay Rate: $67 per hour

Roles & Responsibilities

Provide compassionate end-of-life care to patients in their homes
Manage comprehensive hospice cases with skill and empathy
Coordinate care plans that honor each patient's unique journey
Support families through one of life's most meaningful transitions
Document care using McKesson/Netsmart EMR systems

Qualifications

Active Vermont RN license
BLS/BCLS certification
At least 1 year of hospice nursing experience
Experience with hospice case management
Comfort providing care in home settings

Education

Associate or Bachelor's degree in Nursing
Current, unrestricted RN license in Vermont

Knowledge & Skills

Hospice care principles and best practices
Case management and care coordination
Home health nursing skills
McKesson/Netsmart EMR experience
Rural travel and patient navigation
