Roles & Responsibilities

• Provide virtual patient care and assessments
• Develop and implement individualized care plans
• Coordinate with interdisciplinary teams for optimal outcomes
• Document patient encounters accurately in the EHR
• Educate patients and families on health management
• Monitor vital signs and respond to changes promptly

Qualifications

• Registered Nurse license in California
• Experience in telehealth or virtual care preferred
• Strong communication and critical thinking skills
