Shift Type: Monday-Friday, Day shift (5x8-hour) — one of: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Every other weekend required (or 4 weekend days in a 4-week schedule). Day shift.

Roles & Responsibilities

• Coordinate comprehensive case management and discharge planning to ensure safe, timely transitions of care
• Perform utilization review and apply Milliman Guidelines to support appropriate care decisions
• Document care plans, utilization decisions and discharge summaries accurately in Epic
• Collaborate with interdisciplinary teams (PT, respiratory, pharmacy, social work, etc.) to optimize patient outcomes
• Conduct needs assessments and order DME as part of discharge planning
• Provide care coordination across patient age groups—from neonates to geriatrics—ensuring continuity and quality
• Float within the case management service as needed to cover gaps and support department operations

Qualifications

• Active Alaska RN license in hand required for Auto Offer (screen for pending licenses)
• Minimum 1 year acute hospital experience (behavioral health does not apply)
• Epic EMR experience required (hardstop) — experience charting in Epic is essential
• ACM certification preferred
• BLS preferred (note: some notes list BLS required, others preferred; BLS is preferred for this role)
• Ability to work scheduled weekends (every other weekend or equivalent)
• Locals not accepted — permanent address must be at least 50 miles from the facility

Education

• Active Registered Nurse (RN) license for Alaska (required)
• ACM certification preferred; BLS preferred

Knowledge & Skills

• Case management, discharge planning and care coordination expertise
• Utilization review and familiarity with Milliman Guidelines
• Proficient Epic charting and EMR workflows
• Comfort working in an acute hospital environment (ICU, MS, SDU/PCU/IMC/Observation specialties noted)
• Experience with Pediatrics and Women’s Services is a plus
• Familiarity with Microsoft Office and Teams (preferred)
• Strong interdisciplinary communication and patient advocacy skills
