Roles & Responsibilities
- Provide compassionate hands-on patient care and assist with activities of daily living
- Administer medications and treatments safely and accurately per facility policy
- Monitor and document patient vital signs and clinical changes in the chart
- Collaborate with RNs and interdisciplinary team members to implement care plans
- Communicate clearly and professionally with patients and families about care needs
- Maintain a safe, clean environment and follow infection prevention protocols
- Support admissions, discharges, and transfers to keep unit flow smooth
Qualifications
- Current Licensed Practical Nurse (LPN) license in West Virginia or compact-eligible where applicable
- Current BLS/CPR certification required
- State-required immunizations and ability to pass background and drug screening
- Comfortable working night shift hours and handling clinical responsibilities independently
- Previous experience in long-term care, rehabilitation, or similar settings preferred
Education
- LPN diploma or vocational nursing program completion required
Knowledge & Skills
- Strong medication administration and documentation skills
- Proficient in patient assessment, including vital signs and basic clinical observations
- Familiarity with infection control standards and safe patient handling
- Effective communication and teamwork skills with patients, families, and staff
- Time management and prioritization in a fast-paced clinical environment
- Basic computer/charting skills for electronic medical records
