Roles & Responsibilities

• Provide compassionate, direct patient care and support activities of daily living
• Administer medications and treatments accurately and document per facility policy
• Perform ongoing patient assessments and promptly report changes to the care team
• Collaborate with RNs, physicians, and allied staff to create and follow person-centered plans
• Maintain clear, timely documentation in the medical record and communicate with families
• Support infection prevention and safety standards to protect patients and staff
• Assist with admissions, transfers, and discharges to keep care transitions smooth and safe

Qualifications

• Current Licensed Practical Nurse (LPN) credential — ready to practice
• Active or eligible to obtain Pennsylvania LPN licensure
• Current CPR/BLS certification or willingness to obtain upon hire
• Comfortable working in long-term care/rehab settings and collaborating with a multidisciplinary team
• Strong communication skills and a patient-first attitude

Education

• Graduate of an accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Medication administration and accurate clinical documentation
• Basic patient assessment and vital sign monitoring
• Familiarity with standard infection control practices and safety protocols
• Ability to prioritize tasks in a busy long-term care environment
• Teamwork, empathy, and clear verbal/written communication skills
• Comfort using electronic health records and following care plans
