Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and routine assessments
• Perform wound care, patient monitoring, and assist with activities of daily living as needed
• Collaborate with nurses, therapists, and interdisciplinary team members to implement individualized care plans
• Document assessments, interventions, and patient responses accurately in the medical record
• Educate patients and families on care plans, discharge instructions, and condition management
• Escalate changes in patient condition promptly and communicate effectively during handoffs

Qualifications

• Current, unrestricted LPN license (Licensed Practical Nurse) — RN candidates may be considered based on facility needs
• BLS/CPR certification required (basic life support)
• Experience in rehab, long-term care, or skilled nursing preferred
• Strong clinical judgment, attention to detail, and excellent communication skills
• Ability to work collaboratively in a fast-paced team environment

Education

• Graduate of an accredited practical nursing program (LPN) or equivalent nursing education
• High school diploma or GED required

Knowledge & Skills

• Solid knowledge of medication administration, wound care, and basic nursing procedures
• Comfortable with patient assessments, vital signs, and monitoring changes in condition
• Familiarity with electronic medical records and accurate documentation practices
• Good time management, prioritization, and ability to work as part of a care team
• Patient-centered approach with strong interpersonal and teaching skills
