Shift Type: Days to be determined, 6:30 AM - 7:00 PM day shift

Candidate Pay Rate: $31 per hour

Roles & Responsibilities
- Provide compassionate direct patient care, including medication administration and routine assessments
- Perform wound care, patient monitoring, and assist with activities of daily living as needed
- Collaborate with nurses, therapists, and interdisciplinary team members to implement individualized care plans
- Document assessments, interventions, and patient responses accurately in the medical record
- Educate patients and families on care plans, discharge instructions, and condition management
- Escalate changes in patient condition promptly and communicate effectively during handoffs
Qualifications
- Current, unrestricted LPN license (Licensed Practical Nurse) — RN candidates may be considered based on facility needs
- BLS/CPR certification required (basic life support)
- Experience in rehab, long-term care, or skilled nursing preferred
- Strong clinical judgment, attention to detail, and excellent communication skills
- Ability to work collaboratively in a fast-paced team environment
Education
- Graduate of an accredited practical nursing program (LPN) or equivalent nursing education
- High school diploma or GED required
Knowledge & Skills
- Solid knowledge of medication administration, wound care, and basic nursing procedures
- Comfortable with patient assessments, vital signs, and monitoring changes in condition
- Familiarity with electronic medical records and accurate documentation practices
- Good time management, prioritization, and ability to work as part of a care team
- Patient-centered approach with strong interpersonal and teaching skills
