Roles & Responsibilities
- Provide skilled speech-language pathology evaluations and individualized treatment for SNF residents
- Develop and document measurable care plans that support patient progress and safe discharge
- Collaborate with nursing, therapy colleagues, and families to coordinate holistic care
- Monitor and record patient outcomes, adjusting interventions based on progress
- Maintain productivity standards and follow agency policies and facility procedures
- Attend team meetings and contribute to a positive, patient-centered therapy culture
Qualifications
- 1-3 years of SLP experience (posted requirement: 1-3 years)
- Active SLP credentials as required by facility/state or ability to obtain promptly
- Experience working in skilled nursing facility or long-term care preferred
- Strong documentation and care planning skills
- Ability to work on-site and meet facility attendance requirements to receive guaranteed hours
- Willingness to follow Continuum Rehabilitation Group policies and facility dress code (black scrubs)
Knowledge & Skills
- Skilled in assessment and treatment of dysphagia and communication disorders
- Familiarity with SNF workflows and interdisciplinary communication
- Proficient documentation skills and outcome tracking
- Comfortable working with geriatric populations and medically complex cases
- Strong verbal and written communication with patients, families, and staff
- Time management skills to meet productivity expectations
