Roles & Responsibilities

• Conduct thorough speech-language evaluations to set meaningful goals
• Design and implement individualized treatment plans for SNF residents
• Deliver daily therapy sessions focused on measurable progress
• Manage timely and accurate documentation and discharge planning
• Collaborate closely with nursing and interdisciplinary team members to optimize care
• Meet productivity expectations and follow CRG policies and procedures

Qualifications

• 1–3 years of SLP experience (job lists 1-3 years; 1 year minimum)
• Prior SNF (skilled nursing facility) experience strongly preferred
• Active and valid SLP licensure as required by state/facility (verify before assignment)
• Comfortable with evaluations, daily treatments, and discharge planning
• Ability to adhere to facility policies (CRG P&P) and meet a 90% productivity requirement
• Willingness to wear black scrubs and be present in the building to receive guaranteed hours

Knowledge & Skills

• Skilled in speech-language pathology assessments and treatment planning
• Strong clinical documentation and discharge summary abilities
• Experience working in a skilled nursing facility environment
• Excellent communication and team collaboration skills
• Time management and productivity-focused practice
• Resident-centered care and adaptability to varied caseloads
