Roles & Responsibilities
- Conduct thorough speech-language evaluations to set meaningful goals
- Design and implement individualized treatment plans for SNF residents
- Deliver daily therapy sessions focused on measurable progress
- Manage timely and accurate documentation and discharge planning
- Collaborate closely with nursing and interdisciplinary team members to optimize care
- Meet productivity expectations and follow CRG policies and procedures
Qualifications
- 1–3 years of SLP experience (job lists 1-3 years; 1 year minimum)
- Prior SNF (skilled nursing facility) experience strongly preferred
- Active and valid SLP licensure as required by state/facility (verify before assignment)
- Comfortable with evaluations, daily treatments, and discharge planning
- Ability to adhere to facility policies (CRG P&P) and meet a 90% productivity requirement
- Willingness to wear black scrubs and be present in the building to receive guaranteed hours
Knowledge & Skills
- Skilled in speech-language pathology assessments and treatment planning
- Strong clinical documentation and discharge summary abilities
- Experience working in a skilled nursing facility environment
- Excellent communication and team collaboration skills
- Time management and productivity-focused practice
- Resident-centered care and adaptability to varied caseloads
