Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless care transitions for patients across the hospital
Collaborate with physicians and social workers to create effective discharge plans
Utilize Midas and Meditech systems to track and manage patient cases
Advocate for patients while ensuring they receive the resources they need after leaving the hospital
Manage complex discharge planning with compassion and attention to detail

Qualifications

Active RN license in Florida or compact license
At least 1 year of case management experience
Hands-on experience with Midas and Meditech systems
Strong background in discharge planning preferred

Knowledge & Skills

Proficient with Midas and Meditech electronic medical records
Skilled in complex discharge planning strategies
Excellent communication and collaboration abilities
Detail-oriented with strong organizational skills
