Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation for evenings, weekends, and holidays

Hours per Week: 40

Candidate Pay Rate: $62 per hour

Roles & Responsibilities

Conduct initial assessments of clients and families to determine home health needs
Develop and implement care plans based on diagnosis and client needs
Administer medications and treatments as prescribed by physicians
Provide health care instructions to clients and families
Coordinate client care with physicians and other healthcare professionals

Qualifications

Current RN license
BLS certification
1 year of experience in Home Health and Hospice care
OASIS experience
Driver's License and Auto Insurance

Education

Associate's or Bachelor's degree in Nursing

Knowledge & Skills

Home Health and Hospice care
OASIS
BLS
EMR systems (HomeCare HomeBase)
Strong communication and interpersonal skills
