Location: Sacramento, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: 8-hour shifts, Monday-Friday, with potential for 12-hour night shifts for ED experienced candidates. Minimal time off requests expected.

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct comprehensive care coordination and discharge planning
Utilize review and assessment skills to ensure high-quality patient care
Collaborate with multidisciplinary teams to achieve optimal patient outcomes
Develop and implement effective discharge plans
Communicate effectively with patients, families, and healthcare providers

Qualifications

Minimum 3 years of hospital experience in acute level care coordination
Proficient in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
Experience with discharge planning and utilization review
Prior CA Dignity/CommonSpirit acute care experience required
Current RN licensure in California

Education

Associate's or Bachelor's degree in Nursing

Knowledge & Skills

Strong clinical knowledge and critical thinking skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Strong problem-solving and analytical skills
Familiarity with Medicare/Medi-Cal guidelines and SNF placement strategies
