Roles & Responsibilities

Conduct physical therapy assessments and interventions for patients
Provide SOC visits, especially for ortho patients
Utilize EMR system for efficient documentation
Manage patient caseloads with a focus on quality care

Qualifications

Physical Therapy degree
Current Physical Therapy licensure
Home Health experience preferred, but not required
BLS certification required

Education

Physical Therapy degree

Knowledge & Skills

Physical therapy techniques and best practices
Home health care principles
EMR system proficiency
Strong communication and interpersonal skills
