Roles & Responsibilities

• Conduct comprehensive physical therapy assessments for residents with memory impairments
• Develop and implement individualized treatment plans that incorporate art, music, and gardening activities
• Collaborate with interdisciplinary team members to maximize functional outcomes and quality of life
• Document progress using standardized assessments such as BCAT, GDS, and ACL
• Educate family members and care partners on supportive techniques and home exercise programs

Qualifications

• Current Physical Therapist license in Maine or compact state
• Experience working with older adults or memory care populations preferred
• Strong communication and teamwork skills
• Ability to adapt therapy techniques to individualized resident needs

Knowledge & Skills

• Knowledge of geriatric and dementia‑related physical therapy interventions
• Proficiency with standardized assessment tools (BCAT, GDS, ACL)
• Skill in creating engaging, activity‑based therapy sessions
• Understanding of person‑centered care models
