Location: Santa Maria, California

Start Date: 06-17-2026

End Date: 09-16-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; every 3rd weekend; minor holidays as needed

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments in home settings to determine functional status and therapy needs
Develop and implement individualized treatment plans focused on improving mobility, strength, and independence
Educate patients and families on therapeutic exercises and safety strategies for home environments
Collaborate with an interdisciplinary team including nurses, OTs, and SLPs to ensure coordinated care
Document patient progress using Oasis and Cerner EMR systems with accuracy and timeliness

Qualifications

Valid California Physical Therapist license required
BLS certification (American Heart Association) required
Minimum one year of experience as a Physical Therapist; home health experience strongly preferred
Experience with Oasis documentation (federally mandated) is required
Proficiency with Cerner EMR system

Knowledge & Skills

Knowledge of Oasis documentation and regulatory requirements for home health
Skilled in Cerner EMR for clinical documentation
Home health or community-based care experience is a plus
Strong communication and interpersonal skills for patient and family education
Ability to work independently and manage a geographically assigned caseload
