Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Conduct physical therapy assessments and interventions for patients in home health settings
- Develop and implement individualized treatment plans
- Collaborate with nursing staff and other healthcare professionals
- Perform SOC visits, especially for orthopedic patients
Qualifications
- 2 years of experience in Physical Therapy (will consider travelers with less experience and Home Health experience)
- BLS certification required
- Experience with Home Health Physical Therapy preferred
- Ability to work independently with minimal supervision
Education
- Physical Therapy degree
Knowledge & Skills
- Physical Therapy skills
- Home Health experience
- EMR proficiency (HCHB - HomeCare HomeBase)
- Strong communication and interpersonal skills
