Roles & Responsibilities

• Provide physical therapy services to patients in their homes, focusing on orthopedic and geriatric conditions
• Perform start-of-care (SOC) visits, especially for ortho patients when nursing is not ordered
• Develop and implement individualized treatment plans to improve mobility and quality of life
• Collaborate with the care team, including RNs and home health aides, to ensure comprehensive care
• Document patient progress accurately in HomeCare HomeBase (HCHB) EMR system

Qualifications

• Active California Physical Therapy license (or ability to obtain before start)
• BLS certification required
• Minimum of 1 year of Home Health experience (travelers with less than 2 years considered if they have Home Health experience)
• Experience with HCHB (HomeCare HomeBase) EMR preferred
• Ability to travel within Tehama County service area (approx. 40 miles/day mileage)

Knowledge & Skills

• Strong clinical skills in orthopedic rehabilitation and geriatric comorbidities (CHF, COPD)
• Excellent communication and patient education abilities
• Comfortable with independent work in a home health setting
• Proficient in electronic medical records, specifically HomeCare HomeBase
• Time management and organizational skills to manage 5 visits per day
