Roles & Responsibilities

Conduct physical therapy assessments and treatments for patients
Provide SOC visits, especially for orthopedic patients
Utilize EMR system (HCHB - HomeCare HomeBase) on a provided tablet
Manage a caseload of approximately 5 visits per day
Collaborate with Intake RN and Manager in the office

Qualifications

2 years of experience in Physical Therapy (will consider travelers with less experience and Home Health experience)
BLS certification
Experience with Home Health and Physical Therapy
Ability to work independently with minimal supervision

Education

Degree in Physical Therapy (PT)

Knowledge & Skills

Physical Therapy techniques and best practices
Home Health care principles
EMR system (HCHB - HomeCare HomeBase)
Strong communication and interpersonal skills
