Roles & Responsibilities

• Assess patients' functional abilities and develop individualized occupational therapy plans
• Guide clients through therapeutic activities to improve daily living skills
• Collaborate with interdisciplinary team to coordinate comprehensive rehab care
• Document progress notes and adjust treatment plans based on outcomes
• Educate patients and families on home exercise programs and adaptive techniques

Qualifications

• Current state licensure as an Occupational Therapist
• Completion of an accredited OT program (MS or BS)
• Strong communication and teamwork skills
• Experience in rehab, LTC, or home health preferred

Knowledge & Skills

• Knowledge of occupational therapy assessment tools and interventions
• Ability to modify activities for diverse patient populations
• Proficiency in documentation and electronic health records
