Occupational Therapist - Home Health
Location: Santa Maria, CA
Start Date: 06-15-2026
End Date: 09-14-2026
Contract Type: Contract
Shift Type: Flexible shifts, majority Monday-Friday 8:00 AM - 5:00 PM, with every 3rd weekend required
Hours per Week: 40
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Provide home-based occupational therapy services to patients in assigned geographic region
- Develop and implement individualized treatment plans to improve daily living skills
- Complete OASIS documentation accurately and in a timely manner
- Collaborate with interdisciplinary team including PT, SLP, and nursing staff
- Educate patients and families on adaptive techniques and equipment
- Monitor patient progress and adjust treatment plans as needed
Qualifications
- Valid California Occupational Therapist license
- BLS certification required
- Minimum 1 year of occupational therapy experience
- Experience with OASIS documentation required
- Home health experience preferred
Knowledge & Skills
- Proficiency in OASIS documentation
- Knowledge of Cerner EMR
- Strong clinical assessment and treatment planning skills
- Excellent communication and patient education skills
- Ability to work independently in a home health setting
