Contract Type: Per-Diem

Shift Type: Monday-Friday, 2:00 PM - 10:00 PM; every other weekend required

Hours per Week: 40

Candidate Pay Rate: $33 per hour

Roles & Responsibilities

  • Care, feed, and mobilize residents with compassion
  • Follow individualized care plans to align with resident goals and safety
  • Assist with ambulation, transfers, and positioning using gait belts and lifts
  • Provide restorative and range‑of‑motion exercises
  • Respond promptly to call lights and resident needs
  • Support residents with assistive devices and adaptive equipment
  • Assist during mealtimes, including grooming and documentation
  • Observe and report changes in resident status to the Licensed Nurse
  • Communicate effectively with families, nurses, and interdisciplinary staff
  • Document ADLs, intake/output, and restorative care accurately
  • Participate in onboarding and training of new staff
  • Maintain a clean, safe, and organized resident environment
  • Follow infection control protocols and PPE use
  • Report equipment concerns and safety hazards immediately
  • Support a calm, homelike atmosphere and minimize resident stressors
  • Protect resident belongings and report missing items
  • Ensure residents are free from abuse, neglect, and restraints
  • Honor residents’ right to refuse care and seek alternatives
  • Participate in emergency procedures and respond appropriately
  • Assist with dining room setup, social engagement, and community activities

Qualifications

  • 2+ years of experience as a CNA
  • Active BLS certification and Wisconsin CNA license
  • Ability to lift 50+ lbs and support residents safely
  • Computer literacy, including electronic medical records
  • Strong communication and empathy skills
