Roles & Responsibilities

• Provide compassionate care, feeding, and mobility support to residents
• Follow individualized care plans and uphold resident rights, preferences, and safety
• Assist with ambulation, transfers, and positioning using gait belts and mechanical lifts
• Deliver restorative and range‑of‑motion exercises as assigned
• Respond promptly to call lights and attend to resident needs with empathy
• Support residents during meals, including set‑up, grooming, feeding assistance, and documentation of intake
• Observe and report changes in physical, emotional, or cognitive status to the Licensed Nurse
• Maintain accurate electronic documentation of ADLs, intake/output, and restorative care
• Follow infection control protocols, including proper PPE use and hand hygiene
• Keep resident environments clean, safe, and organized, including stocking supplies and linen care
• Participate in care conferences, interdisciplinary communication, and onboarding of new staff

Qualifications

• Active Wisconsin Certified Nursing Assistant license on the Nurse Aide Registry
• Current Basic Life Support (BLS) certification
• Minimum 2 years CNA experience in long‑term care or related settings
• Ability to lift 50+ lbs and safely support residents
• Computer literacy with experience using electronic medical records
