Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents and patients
Administer medications and treatments accurately and document thoroughly
Monitor and record vital signs, reporting changes to the care team promptly
Assist with daily living activities while promoting dignity and independence
Collaborate with nurses, therapists and facility staff to deliver coordinated care
Maintain accurate clinical documentation in the facility charting system

Qualifications

Licensed Practical Nurse (LPN) license in good standing (state licensure required)
Current CPR/BLS certification
Previous experience in long-term care, skilled nursing or similar setting preferred but not required
Strong communication skills and ability to work well on a multidisciplinary team
Reliable, professional and committed to high standards of patient care

Education

LPN diploma, certificate or equivalent practical nursing program completion

Knowledge & Skills

Patient assessment and basic clinical decision-making
Medication administration and safety practices
Vital sign monitoring and early recognition of changes in condition
Infection prevention and safe infection control practices
Clinical documentation and basic electronic medical record familiarity
Effective communication with patients, families and care teams
