Roles & Responsibilities

• Provide compassionate physical therapy services to patients in their homes across greater Sacramento county
• Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
• Help patients regain mobility and independence in the comfort of their own homes
• Collaborate with interdisciplinary teams to deliver exceptional patient-centered care
• Document patient progress and maintain accurate clinical records
• Build meaningful relationships with patients and their families while supporting their recovery journey

Qualifications

• Active Physical Therapy Assistant license in California
• Prior home health experience preferred
• Strong clinical skills across multiple patient populations
• Excellent communication and interpersonal abilities
• Valid driver's license and reliable transportation
