Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a 40-bed long term care unit
• Administer medications and treatments safely and accurately
• Assess and monitor residents' conditions, reporting changes to the interdisciplinary team
• Document care and clinical observations clearly and thoroughly in the EMR
• Collaborate with nursing, therapy and support staff to develop and follow individualized care plans
• Support residents with dementia and memory care needs using best-practice approaches
• Assist with admissions, discharges and transfers to ensure smooth transitions
• Promote a safe environment by following infection control and safety protocols

Qualifications

• Active New York LPN license required
• Current BLS certification required
• Minimum 1 year of LPN clinical experience
• Long term care experience preferred
• Experience with PointClickCare EMR is a plus
• Strong clinical judgment and excellent documentation skills

Education

• Graduate of an accredited LPN/LVN program (or equivalent practical nursing education)

Knowledge & Skills

• Medication administration and medication safety practices
• Skilled clinical assessment and resident monitoring
• Charting and electronic documentation (PointClickCare familiarity preferred)
• Geriatric care, dementia care, and memory care approaches
• Good communication and teamwork skills with residents and families
• Ability to prioritize tasks in a fast-paced long term care setting
