Roles & Responsibilities
- Assess and diagnose speech, language, and swallowing disorders for residents in a skilled nursing setting
- Develop and implement individualized therapy plans that promote functional communication and safe swallowing
- Document progress, outcomes, and compliance with regulatory standards in the electronic health record
- Collaborate with nursing, PT/OT, and physicians to coordinate interdisciplinary care
- Educate patients, families, and staff on communication strategies and dysphagia precautions
Qualifications
- Current state license as a Speech-Language Pathologist in New Hampshire
- ASHA Certificate of Clinical Competence (CCC‑SLP) or equivalent
- Minimum 1 year experience in acute or post‑acute care preferred
- Strong interpersonal skills and ability to work in a team environment
Knowledge & Skills
- Conduct comprehensive speech and language evaluations
- Design evidence‑based treatment interventions
- Document clinical findings accurately and timely
- Utilize assistive communication technology
- Participate in interdisciplinary case conferences
