Roles & Responsibilities

• Assess and diagnose speech, language, and swallowing disorders for residents in a skilled nursing setting
• Develop and implement individualized therapy plans that promote functional communication and safe swallowing
• Document progress, outcomes, and compliance with regulatory standards in the electronic health record
• Collaborate with nursing, PT/OT, and physicians to coordinate interdisciplinary care
• Educate patients, families, and staff on communication strategies and dysphagia precautions

Qualifications

• Current state license as a Speech-Language Pathologist in New Hampshire
• ASHA Certificate of Clinical Competence (CCC‑SLP) or equivalent
• Minimum 1 year experience in acute or post‑acute care preferred
• Strong interpersonal skills and ability to work in a team environment

Knowledge & Skills

• Conduct comprehensive speech and language evaluations
• Design evidence‑based treatment interventions
• Document clinical findings accurately and timely
• Utilize assistive communication technology
• Participate in interdisciplinary case conferences
