Roles & Responsibilities

• Conduct screenings, evaluations, and treatments for patients with speech, language, and swallowing disorders
• Design and update individualized treatment and discharge plans that truly fit each patient
• Document services accurately in compliance with regulations and facility standards
• Partner with interdisciplinary care teams, patients, and families to deliver coordinated care
• Uphold infection control and safety protocols to keep everyone protected
• Track patient progress and celebrate those communication and swallowing wins along the way

Qualifications

• Graduate of an accredited Speech-Language Pathology program (MS or MA)
• Current state SLP license
• Certificate of Clinical Competence (CCC-SLP) or CFY eligibility
• Strong communication skills and a knack for clinical problem-solving
• A team-first attitude and genuine care for patient outcomes
• Comfort working in a skilled nursing facility setting
