Roles & Responsibilities

• Coordinate and manage care for pre- and post-transplant patients on the waitlist
• Perform patient assessments on clinic days and assist with rooming
• Utilize MediTech and other software to maintain accurate patient records
• Participate in team meetings via WebEx to ensure holistic care
• Provide compassionate support to patients and families throughout the transplant journey
• Collaborate with interdisciplinary teams to optimize patient outcomes

Qualifications

• Current RN license in Texas or compact state
• At least 1 year of nursing experience
• BLS certification from American Heart Association
• Proficiency with computer systems and electronic medical records
• Willingness to learn and adapt to an outpatient transplant setting
• A caring, compassionate demeanor with strong communication skills

Knowledge & Skills

• Strong patient assessment skills
• Proficiency in MediTech and Microsoft Office
• Understanding of transplant services (preferred)
• Excellent data entry and computer skills
• Ability to handle complex patient psychosocial needs
• Knowledge of UNOS and OPO processes (nice to have)
