Roles & Responsibilities

• Provide skilled nursing care to adult patients in their homes across Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge counties
• Perform start of care, recertifications, and daily visits - manage a caseload of about 30 patients
• Use Homecare Homebase (HCHB) for charting and documentation
• Collaborate with clinical managers and educators for support and guidance
• Ensure compliance with all state and federal regulations

Qualifications

• Active RN license in the state of Kentucky (or compact license)
• Minimum 1 year of Med-Surg experience in a hospital setting
• At least 1 year of Home Health experience required
• Proficiency with Homecare Homebase (HCHB) charting system
• Current BLS certification

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Comfortable working independently in a home health setting
• Time management and organizational skills to handle 6.5 points per day
