Roles & Responsibilities

• Provide comprehensive acute case management for patients
• Coordinate interdisciplinary care and develop individualized care plans
• Monitor patient progress and facilitate discharge planning
• Educate patients and families on care transitions
• Collaborate with physicians and allied health professionals
• Maintain accurate documentation in Meditech, Midas, and Interqual systems

Qualifications

• Registered Nurse license in Florida
• Minimum 2 years of acute care case management experience
• Proficiency in Meditech, Midas, and Interqual
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
