Roles & Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans
• Provide hands‑on therapeutic interventions to improve mobility and reduce pain
• Document progress notes and communicate outcomes with interdisciplinary team members
• Educate patients and families on home exercise programs and injury prevention
• Collaborate with nursing staff to ensure safe discharge planning

Qualifications

• Current Massachusetts Physical Therapist license
• Bachelor of Science in Physical Therapy or Doctor of Physical Therapy degree
• Minimum 0 years of experience (new graduates welcome)
• Strong communication and teamwork skills

Knowledge & Skills

• Knowledge of skilled nursing facility (SNF) care protocols
• Proficiency in manual therapy techniques and therapeutic exercise
• Ability to assess and modify treatment plans based on patient response
• Familiarity with electronic health record documentation
