Roles & Responsibilities

• Assess and evaluate patients' physical conditions and develop individualized rehabilitation plans.
• Implement therapeutic exercises, manual therapy, and modalities to improve mobility and function.
• Monitor progress, adjust treatment plans, and document outcomes in electronic health records.
• Collaborate with interdisciplinary team to coordinate care and ensure optimal patient recovery.
• Educate patients and families on home exercise programs and injury prevention.

Qualifications

• Current New Hampshire Physical Therapist license.
• Minimum of 1 year experience in skilled nursing or long‑term care setting.
• Strong communication and documentation skills.
• Ability to work independently and as part of a team.

Knowledge & Skills

• Patient assessment and functional evaluation.
• Therapeutic exercise prescription.
• Manual therapy techniques.
• Modalities (e.g., ultrasound, electrical stimulation).
• Documentation and electronic medical record proficiency.
• Interdisciplinary collaboration.
