Roles & Responsibilities

• Provide comprehensive PT care to post‑acute patients, developing individualized rehab plans that promote recovery and independence
• Conduct thorough patient assessments, selecting appropriate therapeutic exercises and modalities to meet specific goals
• Collaborate closely with interdisciplinary team members to coordinate care and ensure seamless transitions
• Educate patients and families on home exercise programs and injury prevention strategies
• Maintain accurate, timely documentation in compliance with regulatory standards

Qualifications

• Licensed Physical Therapist in California
• Strong communication and interpersonal skills
• Ability to work independently and manage a caseload efficiently
• Commitment to evidence‑based practice and continuous learning

Knowledge & Skills

• Patient assessment and evaluation
• Therapeutic exercise prescription
• Manual therapy techniques
• Modalities (e.g., ultrasound, electrical stimulation)
