Roles & Responsibilities

• Conduct comprehensive assessments of residents to identify physical therapy needs
• Develop and implement individualized treatment plans focused on improving mobility, strength, and independence
• Provide therapeutic interventions such as exercises, manual therapy, and gait training
• Monitor and document patient progress, adjusting treatments to achieve optimal outcomes
• Collaborate with nurses, physicians, and other therapists to ensure holistic, coordinated care
• Educate patients and their families on home exercise programs and fall prevention strategies

Qualifications

• Active Physical Therapist license in Illinois
• Doctor of Physical Therapy (DPT) degree
• New grads are welcome to apply; prior SNF experience is a plus but not required
• Strong interpersonal and communication skills
