Roles & Responsibilities

• Screen, evaluate and treat patients to help them regain strength, function and confidence
• Build and update individualized treatment and discharge plans of care that truly fit each patient
• Document services accurately and on time so every detail of progress is captured
• Supervise PTAs and Rehab Techs in line with practice acts, mentoring them along the way
• Collaborate with interdisciplinary care teams and families to keep everyone moving in the same direction
• Champion safety, infection control and regulatory standards in every session

Qualifications

• Graduate of an APTA accredited Physical Therapy program (DPT, MSPT or BSPT)
• Active state PT license, or the ability and drive to obtain one
• Strong communication, documentation and clinical skills
• Flexibility and a positive attitude when adapting to different facilities, schedules and patient needs
• A team-first mindset and a genuine passion for helping people move better and feel better

Knowledge & Skills

• Physical Therapy practice across a skilled nursing and rehab setting
• Patient evaluation and clinical assessment
• Treatment plan development and progression
• Clear, timely patient documentation
• Supervision and mentorship of PTAs and Rehab Techs
• Interdisciplinary collaboration with nurses, therapists and families
• Regulatory compliance and infection control awareness
