Location: Yountville, California
Setting: Skilled Nursing Facility (SNF)
Contract: 13 Weeks
Pay: $65–$68/hour
Start Date: ASAP
We’re looking for a Physical Therapist (PT) to join a Skilled Nursing Facility in Yountville, CA for a 13-week contract assignment.
What You’ll Do
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises, gait training, balance, and mobility interventions
- Monitor patient progress and modify treatment plans as needed
- Help residents improve strength, mobility, function, and independence
- Maintain accurate and timely clinical documentation
- Collaborate with PTAs, nursing staff, and the interdisciplinary team
- Provide compassionate, patient-centered care
What We’re Looking For
- Active California PT license
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- SNF experience preferred
- Strong communication and clinical documentation skills
