Roles & Responsibilities

• Assess and document each resident’s functional status and therapeutic needs
• Develop and implement individualized physical therapy treatment plans to improve mobility and independence
• Collaborate with nursing, occupational therapy, and medical staff to coordinate comprehensive care
• Educate patients, families, and caregivers on safe movement techniques and home exercise programs
• Track progress, adjust interventions as needed, and maintain accurate clinical records

Qualifications

• Current Oklahoma Physical Therapist license (or compact license)
• Minimum of 1 year experience in skilled nursing or post‑acute rehabilitation preferred
• Strong communication and teamwork skills
• Ability to work independently and manage a full caseload

Knowledge & Skills

• Gait training, therapeutic exercise, and functional mobility techniques
• Knowledge of SNF regulations, Medicare documentation, and discharge planning
• Proficiency with electronic health record systems
• Assessment of musculoskeletal and neurologic conditions
