Roles & Responsibilities

• Assess and develop individualized rehabilitation plans for post‑hospital patients
• Provide hands‑on therapeutic interventions to improve mobility and function
• Collaborate with nursing and medical staff to coordinate comprehensive care
• Document progress notes and adjust treatment plans based on patient response
• Educate patients and families on home exercise programs and injury prevention

Qualifications

• Current state Physical Therapist license in Maine
• Bachelor’s degree in Physical Therapy (or Doctorate if required)
• Minimum 1 year of experience in skilled nursing or acute rehab setting
• Strong communication and teamwork skills

Knowledge & Skills

• Knowledge of gait training, therapeutic exercise, and manual therapy techniques
• Ability to evaluate functional mobility and design evidence‑based interventions
• Familiarity with electronic health record documentation
• Understanding of post‑acute care regulations and reimbursement
