Roles & Responsibilities

• Assess and develop individualized treatment plans for skilled nursing facility residents
• Implement therapeutic interventions to improve mobility, strength, and functional independence
• Document patient progress and adjust care plans in collaboration with interdisciplinary team
• Educate patients, families, and staff on injury prevention and optimal recovery strategies
• Participate in discharge planning to ensure safe transition of care

Qualifications

• Current Washington State Physical Therapist license
• Bachelor’s degree in Physical Therapy (or higher) from an accredited program
• Passage of the National Physical Therapy Examination (NPTE)
• Minimum 1 year of experience in a skilled nursing or acute care setting preferred
• Strong communication and teamwork skills

Knowledge & Skills

• Gait and balance training techniques
• Therapeutic exercise prescription
• Manual therapy and modalities
• Electronic medical record documentation
• Regulatory compliance for SNF environments
