Roles & Responsibilities

• Assess and develop individualized treatment plans for residents recovering from surgery or injury
• Provide hands‑on therapeutic interventions including gait training, balance work, and functional mobility exercises
• Document progress notes and communicate outcomes with interdisciplinary team members
• Educate patients and families on home exercise programs and injury prevention strategies
• Participate in discharge planning to ensure safe transition back to community or home

Qualifications

• Current state Physical Therapy license in Maryland
• Completion of an accredited PT program (Doctor of Physical Therapy preferred)
• Strong clinical reasoning and communication skills
• Ability to work collaboratively within a multidisciplinary rehab team
• Commitment to delivering compassionate, patient‑centered care

Knowledge & Skills

• Knowledge of orthopedic and geriatric rehabilitation techniques
• Proficiency in outcome measurement tools and electronic health record documentation
• Skilled in manual therapy and therapeutic exercise prescription
• Understanding of infection control and safety protocols in a skilled nursing setting
