Location: Mishawaka, Indiana
Start Date: 10-08-2026
End Date: 09-01-2027
Contract Type: Travel Contract
Shift Type: Days, Monday-Friday, 8:00 AM - 5:00 PM
Candidate Pay Rate: $54 per hour

Roles & Responsibilities
- Assess and develop individualized treatment plans for elderly participants in the PACE program
- Implement therapeutic exercises and interventions to improve mobility and function
- Collaborate with interdisciplinary team to ensure comprehensive care
- Document patient progress and adjust plans as needed
- Educate patients and families on home exercise programs and safety
Qualifications
- Active Physical Therapist license in Indiana
- Current BLS certification
- Experience with geriatric population preferred
- Ability to pass background check and health screenings
Knowledge & Skills
- Knowledge of PACE model of care
- Geriatric rehabilitation skills
- Excellent communication and teamwork
- Time management and documentation skills
